The form which is used for 5 jawazat related purposes is given below. The name of this form is not known but its is application for applying for completion of services given below. For consideration I just call this form as IQAMA FORM for reader understanding. Whenever you apply for these five things you need to fill this application form and submit it to Jawazat. New Iqama of Dependents/Family in Saudi Arabia
While issue for Exit Re-Entry Visa for your Dependents (Manually)
Getting Iqama of New Born Baby – Born within Saudi Arabia
Iqama of Newly Born Child – Born in another country Saudi Arabia
For doing Procedure of  Naqal Maloomat of Passport
